Case Management for Aged & Disabled
Aged and Disabled (A&D) Waivers are appropriate for children or adults who are disabled due to medical conditions. As in all cases of this nature, individuals must qualify for Indiana Medicaid to be eligible for a Medicaid Waiver.
How does this service work?
Children and adults may become dependent due to illness or injury, when this happens the person and his/her family may need assistance in order to stay in their own home. CareStar Indiana provides service and information to the patient, his or her family and others who may be impacted by the person’s dependence. After the person is enrolled in CareStar Indiana a case manager evaluates his or her needs and will arrange for appropriate services to be provided.
These services may include, but are not limited to:
- Adult Day Services / Adult Foster Care Assisted Living / Attendant Care
- Community Transition Services / Congregate Care
- Home modification / Environmental Modification
- Personal Emergency Response System
- Specialized Medical Equipment and Suppliies
- Identify respite options, community options for transportation, recreation and education
